研究背景:在乳癌病人照護中,提升病人生活品質是重要的目標,而性格特質是能夠預測生活品質的關鍵因素之一。根據理論,完美主義特質對於癌症等慢性疾病而言,是影響疾病適應的風險因子,可能藉由不良的人際互動影響身心健康。完美主義可分為自我導向型、社會規定型及他人導向型三向度,過去研究認為此三向度對人際適應與健康的影響有所差異,然而目前針對癌症病人的相關研究仍相當有限。因此,本研究期望能探索乳癌病人的完美主義三向度如何預測癌症寂寞感與生活品質,以及癌症寂寞感在此路徑中的中介效果。
研究方法:本研究採橫斷式調查,以113位術後六個月內的乳癌病人為對象,使用短版多向度完美主義量表(Multidimensional Perfectionism Scale-Short Form; HF-MPS-SF)、癌症寂寞感量表(Cancer Loneliness Scale; CLS)以及EORTC癌症生活品質核心問卷(QLQ-C30)與乳癌生活品質問卷(QLQ-BR23)進行評估,並使用路徑分析檢驗多向度完美主義、癌症寂寞感與生活品質的關係。
研究結果:路徑分析顯示自我導向型完美主義可透過癌症寂寞感正向預測生活品質,社會規定型完美主義可透過癌症寂寞感負向預測生活品質,他人導向型完美主義則無顯著預測效果。不論是自我導向型或社會規定型完美主義,對生活品質的預測皆受到癌症寂寞感的完全中介。
討論與結論:完美主義中的社會規定型向度是預測乳癌病人生活品質的重要風險因子,且癌症寂寞感是其中的關鍵機制,自我導向型則具備部分適應性。當病人過度擔憂外在評價,自覺必須在他人面前表現完美,將使其在面對乳癌與治療的挑戰時,較容易感到與他人疏離、缺乏所需的社會支持,並且有較低的生活品質。醫療團隊可考慮在診斷初期針對社會規定型完美主義進行預防性篩檢,將高風險病人轉介病友團體或臨床心理照護資源,以提升病人生活品質。
Background: While optimizing quality of life (QoL) has become a core priority in breast cancer care, specific personality dispositions serve as critical indicators of patient well-being. Perfectionism has been theorized as a risk factor for illness adjustment in chronic diseases such as cancer, potentially affecting physical and psychosocial health through maladaptive interpersonal functioning. Perfectionism comprises three dimensions — self-oriented perfectionism (SOP), socially prescribed perfectionism (SPP), and other-oriented perfectionism (OOP) — which are thought to differ in their effects on interpersonal adjustment and health outcomes. However, empirical research on perfectionism in cancer populations remains limited. This study aimed to examine how the three dimensions of perfectionism predict cancer loneliness and QoL in breast cancer patients, and to investigate the mediating role of cancer loneliness in these pathways.
Methods: A cross-sectional design was conducted among 113 breast cancer patients within six months post-surgery. Participants were assessed using the Hewitt-Flett Multidimensional Perfectionism Scale-Short Form (HF-MPS-SF), the Cancer Loneliness Scale (CLS), the EORTC Quality of Life Core Questionnaire (QLQ-C30), and the Breast Cancer Module (QLQ-BR23). Path analysis was conducted to examine the relationships among multidimensional perfectionism, cancer loneliness, and quality of life.
Results: Path analysis revealed that SOP positively predicted quality of life through cancer loneliness, while SPP negatively predicted quality of life through cancer loneliness. OOP showed no significant predictive effects. Cancer loneliness fully mediated the relationships between both SOP and SPP on quality of life.
Discussion: SPP emerged as a significant risk factor for quality of life in breast cancer patients, with cancer loneliness serving as the key mediating mechanism, while SOP demonstrated partial adaptiveness. When patients are overly concerned about external evaluation and feel compelled to appear perfect in front of others, they are more likely to experience social disconnection and insufficient social support when facing the challenges of breast cancer and its treatment, resulting in poorer quality of life. Healthcare teams are encouraged to implement proactive screening for SPP upon diagnosis, referring high-risk patients to peer support groups or clinical psychology services to improve quality of life outcomes.
