本研究以 2000 年至 2025 年間臺灣國外醫學與牙醫資格認定的制度變遷為例，探討國際牙醫資格如何逐步形成爭議，以及跨國教育如何改變醫療專業再生產秩序。研究採質性方法，訪談 20 位本土與國際牙醫師、牙醫學生、立法人員及相關人士，並進行兩次田野觀察，結合法規、立法院紀錄、政府文件、新聞與網路資料重建制度變遷的過程，並分析 2022 年《醫師法》附帶決議與 2024 年偏鄉二期計畫。研究發現，臺灣牙醫資格制度由名額管制與資格轉換兩套邏輯共同運作；在跨國教育擴張後，名額管制延伸至臨床實作階段，形成等待位置與制度矛盾，並進一步發展為資格議題場域。爭議形成之後，正式的專業代表位置、代表的正當性與集體行動能力逐漸分裂，資格邊界也成為不同專業群體與治理單位競爭與調整的對象。本文指出，跨國教育重構專業再生產的邊界，並改變牙醫專業維持自主性與壟斷的形式。

This study examines institutional changes in the recognition of foreign medical and dental qualifications in Taiwan from 2000 to 2025. It investigates how the credentialing of international dental graduates gradually became controversial and how transnational education reshaped the order of medical professional reproduction. Using qualitative methods, the study draws on interviews with 20 local and international dentists, dental students, legislative personnel, and other relevant actors, as well as two field observations. It also reconstructs institutional change through laws and regulations, Legislative Yuan records, government documents, news reports, and online materials, with particular attention to the 2022 supplementary resolutions to the Physicians Act and the Ministry of Health and Welfare’s Phase II Program for Enhancing Rural Healthcare (2024–2027).
The findings show that dental credentialing in Taiwan has been governed by two institutional logics: quota control and credential conversion. As transnational dental education expanded, quota control was extended to clinical training, creating a waiting position and institutional contradictions that further developed into an issue field centered on credentialing. After the controversy emerged, professional representation became increasingly fragmented in terms of formal representative positions, legitimacy, and collective action capacity, while credential boundaries became objects of contestation and adjustment among professional groups and governing bodies. This study argues that transnational education has not dissolved the social closure of the medical profession. Instead, it has reconfigured the boundaries of professional reproduction and altered the forms through which the dental profession maintains professional autonomy and monopoly.
